4 your occupation/ If you are self-employed (or receive 1099-NEC/MISC) or you started a new business in 2025, please mention in the email.

5 Please state if you a return client from last year or you are a new client

6 Please state your preferred Date and time ( etc. Jan 21 between 1-5pm, or Any Monday Morning etc. ) for your appointment.

7 If you need to ADD/ or Bring your Children, Your Parents, Your friend or your spouse ( filing separately) 's tax returns with your appointment, please add their information from Number 1- 5 above.

Please note how many tax returns on your appointment if you bring your children, parents, or friends with you.
